With the submission of each Facility Use Application, the applicant verifies authorization to act as the legal agent of the named organization for the purpose of application for and use of an 澳门开奖网 facility. The applicant acknowledges receipt of the 澳门开奖网 Regulations & Policies听and agrees to the conditions of building use as specified herein. The applicant agrees that it shall be responsible for the conduct and control of all participants and spectators, and shall ensure that use of the facility is in compliance with all applicable federal, state, municipal, and 澳门开奖网 safety regulations. The applicant agrees to provide adequate adult supervision during the use of the facility.

The applicant recognizes that the School District鈥檚 property and general liability insurance policies do not provide coverage to external organizations using school facilities.听听A Certificate of Insurance and Endorsement must be provided to complete the application process to Use of Facilities. The following are the requirements:

- GENERAL LIABILITY: $1,000,000 Each Occurrence 鈹 $50,000 Damage to Rented Premises 鈹$1,000,000 Personal & Adv Injury 鈹 $2,000,000 General Aggregate 鈹 $1,000,000 Products Comp/OP AGG
- SEXUAL ABUSE/MOLESTATION should be included in the coverage under the general liability and stated under the Descriptions of Operations. If excluded, it is required to be added to the policy ($1,000,000 Per Occurrence / $2,000,000 Aggregate.)
- ADDITIONAL INSURED ENDORSEMENT LANGUAGE: 鈥湴拿趴蓖, its Board of Trustees, officers, agents, employees, and volunteers are named as additionally insured on this policy pursuant to written contract, agreement, or memorandum of understanding. Such insurance as is afforded by this policy shall be primary, and any insurance carried by District shall be excess and noncontributory.鈥 Additional Insured Endorsements are required to accompany Certificates of Insurance. Certificate of Insurance shall provide thirty (30) day prior written notice of cancellation.
